Como norma, los seres humanos tienen 23 pares de cromosomas que contienen la información necesaria para mantenerlos con vida y que establece sus rasgos fisiológicos. Uno de esos pares determina el sexo de cada individuo: las mujeres tienen dos cromosomas X y los hombres un X y un Y. Esa diferencia, que está detrás del desarrollo de distintos órganos sexuales y las variaciones que estos provocan en el resto del cuerpo, puede explicar, en parte, por qué los hombres suelen tener peor pronóstico cuando tienen un cáncer e, incluso, por qué suelen vivir menos. Hoy, dos artículos publicados en la revista Nature ofrecen nueva información sobre el papel del cromosoma masculino en la vulnerabilidad a distintos tumores.

Uno de esos estudios, liderado por Dan Theodorescu, director del área de oncología del hospital Cedars-Sinai de Los Ángeles (EE UU), se ha fijado en lo que sucede cuando las células pierden el cromosoma Y, algo que pasa en hasta el 40% de los tumores de vejiga y que es más frecuente con el paso de los años. En primer lugar, el equipo recogió información de 300 pacientes con cáncer de vejiga y observó una correlación entre la pérdida del cromosoma y un peor pronóstico. Después, para tratar de comprobar que la ausencia explicaba la agresividad del tumor, utilizaron células de ratones. Los tumores de esos animales en los que había desaparecido el cromosoma Y crecieron más rápido y también se vio una relación similar en células de las que se había sacado el Y con edición genética.

A continuación, los autores del estudio colocaron células tumorales en una placa donde no las expusieron a células inmunes. Además, se las inyectaron a ratones modificados para no tener linfocitos T, encargados de controlar la proliferación descontrolada de células que provoca el cáncer, y a otros con los linfocitos operativos. Ni en el cultivo sin células inmunes ni en los ratones inmunosuprimidos hubo diferencia en el crecimiento tumoral en presencia o ausencia del cromosoma Y. Sin embargo, entre ratones sanos, la falta del cromosoma hizo que el cáncer fuese mucho más agresivo. “Estos resultados nos dicen que cuando las células pierden el cromosoma Y, agotan los linfocitos T, y sin ellas para combatir el cáncer, los tumores crecen de forma agresiva”, afirma Theodorescu en una nota de su institución.

Como buena noticia, los resultados del equipo de Cedars-Sinai sugieren que, tanto en ratones como en humanos, la pérdida del cromosoma Y, además de hacer los tumores más agresivos, los volvía más vulnerables a un tipo de inmunoterapia que devuelve a los linfocitos T su capacidad para destruir el cáncer. Para el investigador del Centro Nacional de Investigaciones Oncológicas (CNIO) Óscar Fernández-Capetillo, el aspecto más interesante del estudio es su aplicación práctica. “Se conocía la pérdida del cromosoma Y como una especie de marca, pero no se sabía cómo afectaba al desarrollo de los tumores. Este grupo demuestra que las células tumorales que pierden el Y apagan el sistema inmune que va a comérselas, pero ven también que esos tumores responden mejor a la inmunoterapia”, apunta. “Este trabajo ayuda con el gran dilema de identificar qué pacientes van a responder a la inmunoterapia”, añade. No obstante, advierte que “se conoce que existe ese efecto, que el tumor con el cromosoma Y perdido responde a la inmunoterapia, pero no se sabe cuánto y se tendrá que estudiar”.

Juan Ramón González, investigador del Instituto de Salud Global de Barcelona (ISGlobal), centro impulsado por la Fundación “la Caixa”, publicó en 2020 un estudio epidemiológico con datos de 9.000 hombres en el que se observó que la pérdida de función de seis genes clave del cromosoma Y incrementaban el riesgo de desarrollar cáncer. “En este estudio que se publica hoy se demuestra fehacientemente el vínculo entre la pérdida del cromosoma Y y la desregulación del sistema inmune en la agresividad del cáncer de vejiga. También se observa que la pérdida del cromosoma Y está afectada por la metilación [una especie de notas que se acumulan sobre el texto del libro del genoma con que nacemos y que modifica la expresión de nuestros genes]. En esta línea, nosotros hemos visto que hay factores ambientales que influyen y otros autores han demostrado que los fumadores, por ejemplo, tienen más pérdida del cromosoma Y. Las consecuencias terapéuticas de este trabajo son inmediatas. Al reestimular las células T por intervenciones conocidas es posible reducir la agresividad el cáncer”, explica González.

Su equipo está trabajando ahora con la empresa TruDiagnostics en la búsqueda de biomarcadores similares para mujeres, pero en el cromosoma X, que cuando tienen una activación incorrecta de su copa extra del cromosoma X también reaccionan peor ante el cáncer. Los autores del artículo de Nature también apuntan a la existencia de genes del cromosoma X relacionados con los del cromosoma Y que pueden tener interés terapéutico.

En un segundo artículo que aparece también este miércoles en Nature, un equipo liderado por Ronald De Pinho, del MD Anderson Cancer Center, en Houston (EE UU), cuenta cómo ha estudiado la influencia de un gen del cromosoma Y en la mayor gravedad del cáncer de colon en los hombres. Utilizando ratones, vieron que el KRAS, un gen responsable de la transformación de células normales en cancerosas, incrementaba la actividad del gen KDM5D del cromosoma masculino, algo que facilitaba la progresión del tumor y dificultaba su detección por el sistema inmune. En un tiempo en que se aspira a que los tratamientos de la multitud de tipos de cáncer sean casi individualizados, trabajos como los publicados en Nature ayudan a considerar la diferente reacción de hombres y mujeres ante estas enfermedades para precisar mejor el diagnóstico y el tratamiento.
